Information to record
Use these prompts to agree the record for your project. Keep the answers and their source references together; leave an unknown value explicit rather than filling it with an assumption.
- Term
- Record the word or label used in the schedule.
- Project meaning
- Explain what the team intends by it.
- Evidence prompt
- Identify the note or evidence needed alongside it.
- Allowed alternatives
- Document agreed equivalent wording, if any.
- Uncertainty approach
- Explain when free text or review is required.
Checks for this task
- Review the vocabulary with the team before fieldwork.
- Avoid mixing an observed feature and a diagnosis in the same label.
- Test an early sample for ambiguous or inconsistent terms.
A worked example
Illustrative example, not a client case: Two recorders use different phrases for the same observation category. The reviewer agrees a project term, records both earlier phrases in the vocabulary note and checks affected entries without silently rewriting their meaning.
Scope and limits
A project vocabulary is not a diagnostic reference or a replacement for professional interpretation.
Use the worksheet
The download contains the prompts and checks above with space for your project notes. It is a plain text document you can edit in a text editor or copy into your project brief. It contains no client information and does not run calculations, import records or change an account.
