Information to record

Use these prompts to agree the record for your project. Keep the answers and their source references together; leave an unknown value explicit rather than filling it with an assumption.

Tree and visit
Identify the asset and observation date.
Feature location
Describe where on the tree the observation relates.
Observation
Record the visible feature in clear words.
Evidence reference
Link photographs or other supporting records.
Interpretation reference
Keep any assessment conclusion identifiable.

Checks for this task

  1. Check that the feature can be located from the note.
  2. State uncertainty instead of converting it into a diagnosis.
  3. Keep the recommendation in its own traceable entry.

A worked example

Illustrative example, not a client case: A note for T005 describes a feature on the lower trunk and references photo P2. The practitioner’s interpretation and recommendation appear separately, so the reader can follow the reasoning from evidence to advice.

Scope and limits

Examples do not diagnose tree conditions or assign risk; interpretation remains with the responsible practitioner.

Use the worksheet

The download contains the prompts and checks above with space for your project notes. It is a plain text document you can edit in a text editor or copy into your project brief. It contains no client information and does not run calculations, import records or change an account.
