Information to record
Use these prompts to agree the record for your project. Keep the answers and their source references together; leave an unknown value explicit rather than filling it with an assumption.
- Recommendation ID
- Give the advice a traceable reference.
- Tree coverage
- Identify the tree or defined group involved.
- Proposed action
- Retain the practitioner’s intended wording.
- Reason and evidence
- Reference the assessment and relevant photographs.
- Conditions or follow-up
- Keep qualifications and additional advice visible.
Checks for this task
- Check the action matches the tree and recorded assessment.
- Do not strip conditions when copying advice into a schedule.
- Ask the practitioner to clarify wording that the recipient cannot interpret.
A worked example
Illustrative example, not a client case: Recommendation R08 for T014 references the assessment and photo P6. The action register copies the reference and preserves the condition attached to the advice, rather than circulating an abbreviated instruction alone.
Scope and limits
The worksheet organises issued advice; it does not generate pruning, treatment or removal recommendations.
Use the worksheet
The download contains the prompts and checks above with space for your project notes. It is a plain text document you can edit in a text editor or copy into your project brief. It contains no client information and does not run calculations, import records or change an account.
