Information to record
Use these prompts to agree the record for your project. Keep the answers and their source references together; leave an unknown value explicit rather than filling it with an assumption.
- Decision needed
- State the question requiring an answer.
- Evidence source
- List the drawings, records or advice relevant to it.
- Responsible role
- Identify who can provide or assess the information.
- Due point
- Record the project stage when the answer is needed.
- Outcome record
- Note the agreed action and any remaining limitation.
Checks for this task
- Circulate the current drawing and report references with the questions.
- Separate a proposed option from an agreed professional conclusion.
- Record actions and circulate the decision register after review.
A worked example
Illustrative example, not a client case: A meeting discusses retaining a tree but the underground works route is unresolved. The register assigns the design input to the relevant team member and leaves the impact question open until the consultant can review that information.
Scope and limits
Meeting notes do not amend issued professional advice or authorise works unless the responsible process explicitly does so.
Use the worksheet
The download contains the prompts and checks above with space for your project notes. It is a plain text document you can edit in a text editor or copy into your project brief. It contains no client information and does not run calculations, import records or change an account.
