Information to record
Use these prompts to agree the record for your project. Keep the answers and their source references together; leave an unknown value explicit rather than filling it with an assumption.
- Tree ID
- Identify the affected asset.
- Earlier reference
- Link the preceding visit or advice.
- Change type
- Distinguish observation, correction and revised recommendation.
- New evidence
- Identify the dated source for the change.
- Reviewer and issue
- Record who reviewed it and which output changed.
Checks for this task
- Retain earlier observations rather than overwriting their history.
- Do not treat a new photograph as a revised recommendation by itself.
- Update connected outputs only after the change is reviewed.
A worked example
Illustrative example, not a client case: A repeat visit adds photographs for T023. The change log records new evidence. A later reviewed recommendation is entered separately, preserving the distinction between collected material and professional advice.
Scope and limits
The worksheet documents change; it does not calculate deterioration, risk or inspection intervals.
Use the worksheet
The download contains the prompts and checks above with space for your project notes. It is a plain text document you can edit in a text editor or copy into your project brief. It contains no client information and does not run calculations, import records or change an account.
